The Chicago Collective will reimburse you $50 per night in any one of the hotels listed below.  This offer is valid only for the nights of January 29–31, 2011.

In order to be eligible for reimbursement, you must make your own hotel reservation through MMPI Travel Services:  Toll Free: 1-800-528-8700 or online at www.chicagocollectiveonline.com.

 
  1. Reimbursement is valid only for the hotels listed below. Please be sure to make your reservation BEFORE submitting this form.
  2. In order to receive a reimbursement check you must fill out this form completely. You will receive a confirmation number via email once your form has been received and processed.  Please allow 72 hours.
  3. You must present a copy of your hotel receipt at the show to claim your reimbursement check. 
  4. Reimbursement checks can be picked up at the show office only on the last day which you are present at the show.
  5. Checks that are not picked up by 3:00 p.m. on February 1st will be forfeited.
